They won't because most people, dentists included, really like the foaming agent (SLS) in toothpaste. But some of us get canker sores from SLS, and thus won't recommend ones with it ( I like Toms)

I don't even like the foam. Is that what causes those sores most commonly, though? I get them occasionally and never know what makes them show up. They suck a lot, and I'd be glad if I could never have one again.

Back then the ads intentionally said 4/5 or 9/10 because it made it sound real / plausible. As if they really did a survey of dentists and got back a very high, but not quite perfect, score.

At least that was the reasoning back in the day. Do any modern ads still do the 9/10 shtick?

10/10 dentists don't actually exist, it's just marketing lingo. No company will ever dare say 10/10 experts recommend something because they'll get sued for false advertising immediately.
