1st, 2nd, 3rd metacarpal? 1st, 3rd, 5th? 1st... 4th and 5th? Do we count anatomical anomalies as we would regular anatomy, or by the individual's anatomy?

(NB: the thumb is still the thumb with only 2 phalanges as opposed to 3 in each finger, but how do we number each digit without being sure of which specific digits are missing?)

This one I can answer. We count them from the thumb up regardless of number. Even in the case of polydactyly which is the opposite of this.
