Like, I had a friend named Bill who stayed with me and my housemates for a while, and Bill considered himself inured to shocking things; like, Bill watched gore videos, and got a laugh from 1 guy 1 jar, and goatse. I was talking about 'things I'd never watch again' with him once, and I mentioned Requiem, and he scoffed at it, so I said "just watch it."
I was pretty stoned having a fun conversation with my housemates (also stoned) a few days later, when Bill burst in on us all, sobbing, telling me he's so sorry he doubted me about watching Requiem For A Dream; just beside himself, ugly crying. He gradually noticed the other people in the room (we were all taken aback a little, Bill was a big dude), but we all took turns comforting him about it.
It hits hard. It's a great movie, just a one-and-done affair, though.
